Forgot Password: Request a Temporary Session

If you have forgotten your Professional Learning Communities Assessment coordinator password, you can use the form below to request a temporary logon session, after which you can change your password to something memorable.

Directions:

1. Use the form below to trigger a temporary session for your user account (e-mail address).
2. An e-mail will be sent to your e-mail address, and the e-mail will contain a link that will log you on to the PLC survey admin site with a temporary session.
3. Click the link, and you will arrive at the PLC administration site logged on as you.
4. You will see a message containing a link that will allow you to set a new password.
